Lake Powell Resorts & Marinas requires all applicants (except internationals) to submit a background authorization form.
You may pick this up at the HR office or simply click here.
Please fax or mail the form to the appropriate office listed below.
Please take note that applications will not be considered unless accompanied by a completed background form
You must supply current physical address and any previous addresses within the last 7 years, any former names used,
current license number and issuing state, and your Social Security number.
